I'm playing at max difficulty and when I whacked a clannfear runt with a 6x sneak attack I nearly killed myself from the 20% reflected damage. I had full health [Redguard with 85 Endurance at level 5] and hit with full power blade (Blade skill at at 86, Strength at 85, using Honorblade with base damage listed at 20.) I looked on the reflected damage wiki page but didn't see any comment about difficulty.

I assume it would be, yeah. The damage multipliers are calculated from a base value, so if it gets reflected it probably recalculates.

I've noticed this too. It certainly seems to be the case that the damage is increased.
