According to the Wiki, Peakstar says she died fighting an Ash Vampire. I haven't confirmed this in-game yet but maybe soon.

Ah! Sorry, what I meant to say was that I was wrong when I said some NPCs claimed she died under temple imprisonment (not sure if my mind made that up completely or if another npc will say that). So far I've only been able to confirm that one NPC, Sharn, believes that the Ordinators are lying since they didn't show proof. I'll have to refresh my memory with a second playthrough. I've no doubt the wiki is correct that she was slain by an ash vamp, but I'll try to double check that too. Yikes, my memory keeps betraying me.

But I'm fairly sure the "holy men scorned my name, forging lies to cleanse my fame" part will hold true.
