Pilgrims can be seen during a random encounter in Northern and Southern Elsweyr, where they are wandering the road and are suddenly attacked by a terror bird. You can kill the terror bird, and also pickpocket the Pilgrim.

When the encounter begins, they'll look around and say something. As the terror bird approaches, they'll run away.

"I knew I had a terror bird following me! I hate being right!"
